Hmm. Where our incoming fibre is secured to the house wall (high up) there is a joint of some sort. Certainly no box.

The contractor that did mine said that they weren't allowed to work in lofts (or drill through plastic). However he did ask if I was happy to run the fibre through the loft to the desired location. As luck would have it, I was, otherwise it would have been impossible to install the ONT in (for me) the best location.

No. there is no high level joint
In my case the fibre runs overhead to an attachment point and then down the eaves and house corner then under the drip bead to the final ground level termination box
It is continuous from the pole to that termination box
I don't plan to get a ladder out:-)

Umm. The original telephone line was retained by being routed through a spring like wire attached to a wall fixing but then continued through the loft to a eaves cupboard and the router. There must be some sort of strain relief for the fibre coming in from the roadside pole. The pole itself now has several junction boxes as data feeds split off to the various properties.

There is. The cable is a bundle consisting of (in my case) a sheathed optical fibre, a large steel cable laid alongside it and a copper pair off to one side, and then an overall sheath,
The full cable was wound onto a spiral so the steel takes the strain. Then the outer sheath was stripped off and cut back with the steel cable, leaving an insulated copper pair and the sheathed fibre. The twin copper goes to a standard phone junction box and the fibre carries on down to the external termination box, where an internal cable is spliced to it.

Well if you are connected via a numpty class 'consumer' ISP...But one assumes most UK.d-i-y people would be on one of the better supported independents.

Plusnet isn't bad, considering that it is part of BT. It knows about fixed IP addresses, so I wouldn't call it 'consumer'. Demon went downhill fast as soon as it was sold, though it wasn't completely destroyed until Vodafone got hold of it.
