Goalsetter Recall

I don't want to sound insensitive to the death and the injuries, but I don't get it. It sounds like it wasn't mounted to the wall right, so why would that be the goal's fault? Are the mounting holes too large, or the metal too soft so the holes get bigger after a while?

Are they misdescribing the problem? The goal breaks in two, with part still attached to the wall and part that falls down?
