It is by no means the first. Pandemics seemed to do alright when the population was many times lower. Air travel is more important factor in rapid sptread than population.

Then there was 'sleepy sickness' (Encephalitis lethargica) a short time later, which killed my grandfather and nearly 5 million others, but small potatoes compared to Spanish Flu.
