Help me identify my kitchen faucet - photo link included

The problem is the hose that runs up through the tall faucet isleaking. I don't know who manufactured the faucet. So unless all thefaucets of this type use the same hose, I need to identify it so I canget the correct hose. Please help! z-

That is a MOEN 87499 Harlon faucet, in case anyone still needs an answer.
