Sorry for the poor quality photo I only had my phone with me at the time I saw this.
I'd love to have seen photos of the scaffolding being erected in the first place. Our H&S rules are based on what might be dangerous for a school kid doing a day's work experience. That said, I know someone who really did have an accident at work while changing a lightbulb. You can't win
