How do you get baseboards to sit flush against the floor without using shoe molding?

I just got back from a trip to the Biltmore Estate and I've been staring at my own living room baseboards with total disgust ever since. The joinery in that place is on another level, expecially considering they were working with stain-grade walnut and oak without any modern power tools. One thing that relly tripped me up was the complete lack of shoe molding or quarter round throughout the entire floor plan. It's just massive, heavy trim sitting perfectly against the floor with zero gaps. I'm trying to figure out how they managed those perfect scribes against the floors without any wiggle room. My own house has some slight waves in the subfloor, and every time I try to scribe a long run of oak, I end up with a hairline gap that I'd normally just hide with a piece of shoe.
