Following some rainwater ingress, the lads came today and replaced the gully between two bits of roof on our house. It seems that the original installers:
1) only used 12mm board instead of 18mm.
2) used code 4 lead sheet instead of code 5 (the former, I'm told, is only supposed to be used for flashing and similar)
3) didn't provide any expansion joints in the lead.
