I see that there is no standard replacement cartridge for a 3/4" bath mixer but is this only because the heads and splines are not standard, i.e. the actual cartridge and its thread and seat are standard dimensions?

Cut your losses and buy a complete tap.
(We're big fans of 1/4 turn taps as SWMBO finds them easier than gripping a tap). But on the plus side, in a soft water area the discs seem to last forever ?????
This is a hard water area.
Yes I had decided to do that but after opening and closing it a few times while out of the mixer body I put it back in and it seems to work.
Now in anticipation of a future failure I'd like to look for a thermostatic mixer which works on a gravity head of about 2.4 metres head, any suggestions?

Most taps, but in particular bath taps, should be made with standard tails that are fixed in place. That way, only the part of the tap that does all the work needs to be replaced, and that's easily accessible.
As an example:
Years of DIY plumbing in the UK has taught me that "standard" and "should" are very much moveable feasts and probably best ignored.

I installed a Screwfix ceramic cartridge bath mixer tap. It is fine as it is obvious which way you have to move the levers to turn the water off. Elsewhere I have a ceramic cartridge mixer tap with round knobs which annoys me as I can never remember which way to turn the knobs. The hot tap is on the left and has to be turned off anticlockwise.
I was pleased to find that the mixer tap shower attachment uses the same size thread on the three taps that I have.
Who replaced the cartridges with wrong handed replacements?
