While replacing helical and worm gears,I shorted out the board and parent company doesn't have it any longer.It's a #556 Overhead Door brand.

Someone at sci.electronics.repair might have it.
If the motor head contains the board you can get the whole works at

on ebay for $49.95 with free shipping.
And from now on you will unplug things before you take them apart.
