When your driveway sits on a slope and starts migrating toward the neighbor's property, those cracks are ussually a sign of a much bigger problem underground. If you're able to slide a screwdriver deep into a gap with zero resistance, you've got a major void where the soil has washed out. Don't just dump sand down there thinking it'll stay put. The next heavy rain will just carry that sand away and make the sinkhole even worse than it was before. What you relly need to do is look into using an expanding structural foam to fill that empty space and stabilize the ground. You drill small holes and pump the material in untill the cavity is packed tight. Once the foundation is solid again, use a closed-cell backer rod pushed about a half-inch down into the crack before applying a high-grade polyurethane sealant over the top. This keeps the water out so the erosion stops. It’s a bit of a project, but it’s the only way to keep your concrete from sliding down the hill.

I dealt with this exact issue on my property in the foothills last October. My slab had dropped about two inches on the west side, and that screwdriver test you mentioned realy opened my eyes to how much dirt had actualy vanished. I ended up renting a hammer drill and went with a two-part foam kit from a local supplier. Drilling those 5/8-inch holes felt like I was ruining the slab, but once that foam started expanding, you coud actually hear the concrete creaking as it stabilized.
One thing I learned the hard way is to watch the temprature. If the ground is too cold, the foam doesn't expand nearly as much, and you'll burn trough your supply way faster than the calculator says. I used about three times more material than I originally budgeted for. For the finish work, I used the Sika Self Leveling Sealant over some 3/4-inch backer rod. It’s held up through three major storms so far without any new sinking. It isn't a cheap fix, but it's way better than watching my driveway slowly crawl toward the street.
Spot on with the foam approach. Just make sure you don't skip cleaning the actual crack walls before you lay that sealant. I found that if you don't hit the edges with a stiff wire brush and blow out the dust, the polyurethane won't acutally bond to the concrete. It’ll look good for a month, but then it just peels away like a sticker. I ussually run a shop vac or a leaf blower right before the backer rod goes in. If that bond isn't perfect, you're just inviting water to get back under there and undo all that expensive foam work.
If you want that sealant to actualy last thru a freeze-thaw cycle, skip the hardware store stuff and get some Vulkem 45SSL. It stays much more flexible than the standard brands and bonds like crazy to the concrete. For the drilling, I wouldn't bother with a rental; just buy a Hilti TE 30-C. It’s got the vibration dampening you’ll want if you’re drilling twenty holes in a row. Also, grab a pack of MasterSeal P 173 primer for the edges before you pour the Vulkem. It’s an extra step, but it’s the only way to guarentee it won't pull away when the slab shifts.
The foam solution sounds solid for a quick fix, but have you ruled out a failing retaining wall or poor gutter drainage first? If your gutters are just dumping water right at the edge of the slab, no amount of structural foam is going to stop that soil from eventually washing out again. I'm also a bit skeptical about the long-term lift on a slope. If the weight of the slab is alredy shifting, is it posible the foam coud acutally act as a lubricant if the underlying drainage isn't fixed? Just seems like you might be treating the symptom rather than the source.
