Yes but when it was in-house Estates would simply equip a bloke with a bulb-laden trolley and a pair of steps and tell him to tour the entire hospital changing any faulty bulbs.

They probably would change *all* the bulbs in case. I think street lights were serviced in that way before LEDs.

It's what most management companies do now (minus the trolley)
No-one sends out a man to replace one bulb
they either have someone who is already on site doing something else, do it, or wait until there is a critical mass and replace a dozen on one visit

You then have to pay for that in house office that orders up the bulbs and steps etc. Of course being a council they'll likely keep all the office staff while outsourcing the trades.
