Which is why it's wrong to emphatically say up or down. It's only close inspection that will reveal whether the shower connection has male threads or the elbow connection.
To me, and based on previous experience, it looked like a standard compression elbow joint. My last electric shower had a plain 15mm stub for connecting to. Hence I guessed "down" but advised close inspection.

Indeed. In my initial reply, I suggested that it could be any of three types[1] of elbow, because it wasn't possible to identify the exact type from the photo.
[1] Compression/compression elbow onto a plain shank with an olive / compression to FI elbow onto a threaded shank / bent tap connector onto a threaded shank. the direction of unscrewing would be clockwise for the first, and anti-clockwise for the other two, with the added proviso that the FI elbow would require the whole fitting to be rotated.
Not at all amazing when you have one photo from a fixed angle to look at.
Indeed, it was a bit tricky. That doesn't excuse giving a definitive answer based on inconclusive evidence though, especially after the conclusion had been revealed! ;-)
Of course I'm only being a smug git because I got it right but it does highlight the dangers of offering advice based on incomplete info.

Well in this case it did not seem like there was much to go wrong. Presumably the OP would try turning one way, and if it was very stiff, try the other anyway. Even if it broke, it sounds like it was an old shower coming out.
