Ideally real. But I know todays price-conscious manufacturers have a "smoke flavour" they will add.
I am certain that a bacon package which displays the word "smoked" means that the contents are smoked.
If it said "smoke-flavoured", it would be different.

manufacturers - aided by government - have gotten very good at mangling language to hide the truth of their products, so the mere presence of "smoked" in a product name wouldn't convince me in and of itself. I'd pore over the entire packaging before being convinced.
There's the weasel word "style" too. You get something "style" <product>
- which is a way of saying "nothing like you'd be expecting".
My current gripe (I almost said beef :) ) with manufacturers is the habit of differentiating quite distinct products with almost invisible markings on the packaging. I know it's not just me as I've seen staff putting things in the wrong section because they missed the tiny label that says "reduced salt" or whatever. It's especially hard for SWMBO whose eyesight is f***ed on a good day.

My understanding is that originally (say, Tudor period and earlier), mince pies did contain minced meat, but the recipe changed - gradually - over the centuries.
There have been any number of historical food programmes on BBC over recent years, usually including Lucy Worsley and Ruth Goodman, which have mentioned this and similar.
The UK has strict food labelling regulations and that includes rules which outlaw misleading information. Smoked bacon is smoked, though perhaps not for as long as it might have been a century ago.
That's alright - that is well-known as a weasel term. A bit like "flavoured", as often seen on crisp packets, etc (no-one is fooled by that, are they?).
The bacon we buy has the doubt-free word "smoked" on the packaging. Not "smokey flavour" or "smokey style". ;-)
I usually look for "reduced fat".
Although I have the usual problems that age brings, I keep my blood pressure under control by never having any salt included in home-cooked food.
