A feeler gauge and a pair of decent side cutters does the job yourself - no need to go to hospital. And no gold lost in the process - unlike cutting or grinding.
Other options:
- Raise hand, well above rest of body. Wait, wait, wait for this to reduce the swelling, try again. Also, use soap as a lubricant.
- Have someone simultaneously push the skin on the finger towards the wrist while pulling the ring towards the fingertip.
From someone in a hospital who had the ring remover, and said he'd never needed it because he used this method instead:
Take string or wool yarn, thin and sturdy. Wrap the finger from tip to base, tightly, compressing the finger as you wrap. Thread the string under the ring, and continue wrapping for a bit. Now unwrap the string -- only now do not go under the ring, but let the string moving around the finger as it unwraps take the ring with it.
(Not an option in burns and cuts, but deals nondestructively with pretty severe swelling...)
