I’ve seen this mistake a dozen times when people are trying to make a home more accessible for an elderly relative. You get so focused on getting a solid grab bar or handrail in place that you forget about basic fire safety or simple room access. If the layout is tight, sometimes it's better to mount a vertical bar on the wall right next to the trim rather than trying to force a diagonal rail where it doesn't belong. It’s all about leverage and keeping the exit clear. Always double-check your clearance before you drive that final screw. People get in a rush and forget that a door actualy has to, well, open. Think about the path of travel and who is going to be using that space in an emergency.

I ran into this exact headache last summer when I was prepping my dad's place. I thougth I could get away with a standard 24-inch Moen bar near the bathroom entrance, but I didn't account for the casing thickness. If I'd gone through with the original plan, the door would've hit the end cap every time it swung past 45 degrees. I ended up having to shift the whole mounting bracket about three inches to the left, which meant opening up the drywall to add some 2x6 blocking because there wasn't a stud where I needed it. For the fasteners, I stopped using the screws that come in the box. They're usally too soft and the heads strip if you hit an old oak stud. I switched to those GRK RSS structural screws. They have a much better bite and I don't worry about them snapping if someone actualy puts thier full weight on the rail during a slip. Getting that solid connection into the framing is the only way I can sleep at night knowing my parents are using those rails. It's easy to overlook how much force a falling adult puts on those points when they lose their balance.
Beyond the GRKs, if you're stuck without a stud, look at the WingIts MAW35 anchors. They're rated for 300 lbs in 5/8 drywall and save a ton of patch work.
Also, watch your projection. I've seen guys install a Moen 8900 and forget that a wheelchair hub will clip the bar every time if the hallway is under 36 inches. If the rough opening is tight, look at the Peisheng low-profile bars or you're just creating a snag hazard for the very people you're trying to help.
While the structural integrity of the fastener is paramount, one must also account for the precise vertical positioning relative to the user's specific anthropometric measurements rather than merely adhering to the standard ADA 33-to-36 inch height specification, wich often fails to accomodate those with limited range of motion in the shoulder girdle. Furthermore, when utilizing high-torque fasteners in aged, high-density framing, I find it absolutely essential to utilize a tapered drill bit for pilot holes to prevent the desiccated grain of the jack stud from splitting under the lateral pressure of the screw threads, as a hairline fracture in the substrate can lead to catastrophic failure under sudden load.
Seen plenty of those 'quick-fix' anchors fail over thirty years. If I can't get behind the wall to install a proper 2x10 bridge between the studs, I still prefer the old-school backer board method. Take a nice piece of 1x6 oak, chamfer the edges so it looks decent, and lag that sucker into the framing. Then you can put your bar exactly where the user needs it, not just where the studs happen to be. It’s alot more solid than trusting a toggle bolt when someone’s acutally taking a header. Too many folks today rely on hardware store gimmicks instead of solid wood-to-wood contact.
