it's 5blade 52" with 4 lights, the glass around the bulbs are 6 sided with metal strips in each corner, label says Taiwan R,O.C. manufacture number 5A0 0040 serial number 551538, have done searches using those numbers, but no results, fan probably 30 years old give or take, one of the arms broke and it's an oddball size

I did a quick search for "five blade ceiling fan blade replacements". A lot of complete sets were under $50. One complete set was $130. Home Depot was one place that had them.
