You would think after the first round, various IT depts would get their shit together and/or shitcan those deemed grossly incompetent in their duties. Further, those in the offices who took the phishing bait and granted access to the malware in the first place by running the attachment should (a) be forced to contribute financially to the clean up costs, as in, payroll deductions/401k etc. and (b) once they've paid whatever amount is considered 'fair' for their participation, they should be released (fired) from said company with a no rehire note attached to their personal file.

Perhaps accountability (no pun intended) would assist with the malware prevention efforts.
