restoring teak veneer table

I have acquired a table that is teak veneer over particle board. It's an OK table but there are gouges in the veneer which expose the particle board. Worse gouge may be 3/8-1/2" long x 1/8-1/4" wide.

Any suggestions on how to fill the gouges and to blend with surrounding veneer?

Also, it looks as though someone overtightened a screw to the point where it shoved some of the veneer up. Can this be moistened and then ironed to bring back to match surrounding surface?

Maybe. Depend on what glue was used and if the screw pushed up (and broke off?) bits of particle board that are still there.
