In the end, owing to lack of space to do anything more ambitious, I have fitted a NRV to the hot water feed from the cylinder. At least this goes some way to stopping the higher pressure mains building up the pressure in the cylinder.

I will fit a PRV in the cold feed to the shower as/when I can get access to the appropriate pipe (there's not really any chance of connecting to the balanced cold feed output from the cylinder PRV). In the longer term I might replace the unvented system with a thermal store (with internal heat exchanger, owing to lack of space).
