what distance from the wall should the toilet waste hole be for a close couple toilet?

I'm not sure whether there is an absolute standard - or whether different toilet/cistern combinations vary. If the toilet has a horizontal outlet - needing a right-angled plastic bend to be fitted, there's slightly more tollerance than if the outlet goes straight down into the floor.

FWIW, I have 3 close coupled toilets (bathroom, en-suite and downstairs cloakroom) and they are all in the region of 100mm from the wall to the centre of the hole.

Depends a bit on what you mean by "distance from the wall". Most toilets have a "P" style trap these days where the waste comes out horizontally at the back and goes through a wall, rather than a "S" type trap where you need a hole in the floor.

The distance from floor to centre line of the outlet connector on most pans is 190mm. The distance from the back of the pan to the outlet connector is not a standard length and will vary from one model of pan to another.
